Participant Responsibilities and Repercussions
Participant Responsibilities:
- Arrive on time for all workshops.
- Attend all scheduled workshop sessions unless unable to do so due to unforeseen circumstances.
- Respect the facilitator and other participants.
- Participate actively and contribute to a positive learning environment.
- Follow the safety guidelines and instructions provided during the workshop.
- Participants are expected to attend the workshop sober. The use of alcohol or drugs during workshop hours, or arriving under the influence, is not allowed.
Repercussions for Non-Compliance: Failure to comply with the participant responsibilities may result in:
- Warning from workshop facilitator.
- Temporary suspension from workshop sessions.
- Removal from the workshop without refund.
Puzzles of Impact Responsibilities:

- Provide necessary materials, tools, and facilities for the workshop.
- Ensure the workshop is conducted in a safe and inclusive environment.
- Facilitate learning and provide support to participants throughout the workshop.
- Adhere to all relevant policies and regulations governing the workshop.
Personal Indemnity
I understand and acknowledge that participation in the workshop conducted by Puzzles of Impact involves certain risks. I hereby agree to the following:
- I assume all risks associated with participation in the workshop, including any physical injury, emotional distress, or other harm that may occur.
- I release, waive, discharge, and covenant not to sue Puzzles of Impact, its facilitators, directors and employees from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in the workshop.

Consent for Personal Information Processing
Puzzles of Impact is committed to protecting your personal information in accordance with the Protection of Personal Information Act (POPIA) of South Africa.
By signing below, you consent to Puzzles of Impact processing your personal information as required for the administration and facilitation of the workshop. Your personal information will be used solely for this purpose and will not be shared with third parties without your explicit consent.
Media and Story Consent
Consent for Use of Photos and Stories
I, consent to Puzzles of Impact using photographs and/or video footage taken during the workshop, as well as my personal story, on social media platforms and other promotional materials.
- I understand that these images and stories may be used to promote Puzzles of Impact’s programs and activities.
- I acknowledge that my participation is voluntary and that I will not receive compensation for the use of my images or story.
Agreement
By submitting this form, I agree to participate in the workshop hosted by Puzzles of Impact and adhere to the guidelines and instructions provided by the facilitators. I acknowledge that I have read and understood the above information and consent to the terms outlined.
